Terms of Agreement

Gift Certificates:

1. Redemption and Scope of Use

• Valid Services: Gift certificates are valid solely for services, testing packages, or consultations offered directly by Health InSight via our online scheduling portal or in-person clinic.

• Online Booking: To redeem a gift certificate, the bearer must enter the unique digital code at the time of scheduling an appointment. We can also enter in the coupon card at the time of the appointment.

• Partial Balances: If the total cost of a scheduled service is less than the value of the gift certificate, the remaining balance will be saved under the original code and can be applied to future bookings. If the service cost exceeds the certificate value, the bearer must pay the remaining balance at checkout.

2. Expiration and Fees

• No Expiration: In accordance with federal law and local regulations, gift certificates do not expire.

• No Maintenance Fees: The Company does not charge activation, dormancy, inactivity, or service fees. The value of your gift certificate remains intact until it is fully redeemed.

3. Price Adjustments and Valuation

• Monetary Value: All gift certificates reflect a fixed dollar amount.

• Service-Specific Certificates: If a gift certificate is purchased for a specific service or testing tier (e.g., a specific metabolic or physiological evaluation), it represents the exact cash value of that service at the date of purchase. If the Company adjusts its service pricing prior to the redemption of the certificate, the bearer is responsible for paying the price differential at the time of booking.

4. Non-Refundable and Non-Transferable

• All Sales Final: All gift certificate purchases are final. Gift certificates are non-refundable and cannot be exchanged, returned, or cancelled for cash value, except where strictly required by applicable state law.

• Resale Prohibited: Gift certificates cannot be resold or transferred to third parties for commercial value.

5. Lost, Stolen, or Fraudulent Certificates

• Code Security: Treat your gift certificate code like cash. The Company is not responsible for lost, stolen, destroyed, or unauthorized use of gift certificate codes.

• Replacement: If a digital code is misplaced, the original purchaser may request a replacement by providing proof of purchase and the email address associated with the original order. The original code will be deactivated upon issuance of a new one.

• Right to Cancel: The Company reserves the right to refuse, cancel, or hold gift certificates for review if we suspect fraud, illegal activity, or a violation of these terms.

6. Scheduling, Cancellations, and No-Shows

• Standard Policies Apply: Appointments booked using a gift certificate are subject to our standard business cancellation and rescheduling policies.

• Late Cancellations and No-Shows: If an appointment booked via a gift certificate is cancelled without notice, or if the client fails to show up for the appointment, the Company reserves the right to deduct the full cost of the scheduled service from the gift certificate balance.

7. Governing Law

These Terms and Conditions are governed by and construed in accordance with the laws of the State of Utah, without regard to conflict of law principles.

By purchasing a Gift Certificate you agree to these terms
